Mild adverse reactions

including bruising, mild tenderness, and edema; serious adverse reactions are rare.

Relative contraindications

including active inflammatory processes, autoimmune diseases (including collagen disorders in remission), blood anticoagulation, liver dysfunction, different types of permanent fillers at the intended injection site or plane, breastfeeding, use of anti-inflammatory or immunosuppressive drugs, recent surgical intervention at the treatment site (within the past two months), and other significant disruptions to homeostasis.

Absolute Contraindications

Active inflammation or infection at the application site, disordered active collagen, permanent
filler in the same location plane, pregnancy, active herpes infection near the treatment area, minors, or mental illness.

Suggested Mixing Method

  1. Rveiling® lyophilized powder specifications: 220mg vial diluted with 2-5ml of sterile water for injection; 360mg vial diluted with 5-8ml of sterile water for injection.

  2. Hydration Time: Let stand in the bottle for 10-30 minutes. Shake manually before injection to ensure even distribution. Use a three-way valve and the "back and forth injection" method.

  3. It is recommended to add 1-2ml of 2% lidocaine to reduce patient discomfort; most patients use 2ml. This can be injected directly into the vial or later into a syringe, maintaining the lidocaine-to-lidocaine ratio at 8:1 or 8:2.

  4. Recommended dosage: Facial care: 8-9ml dilution for 360mg vials; for non-facial areas, 8-16ml is recommended for distribution over a larger treatment area.

  5. The most commonly recommended dosage for each non-facial area is: 10-12 ml for hands; 12-16 ml for neck and chest; and 16-20 ml for buttocks and thighs.


Injection Technique

Needle type

Sharp needle diameter 25~30G, blunt needle cannula diameter 22~25.

Injection method

Preferred injection is retrograde in a linear or fan-shaped pattern.

Injection Depth

For specific cases, perform deeper injection.

Preferred injection is into the superficial subdermal layer; sub-SMAS and near-periosteal layer in specific indications.

Note

Injection around the eyes and lips is feasible, but the risk-benefit ratio should be considered.

The preferred injection layer for PLLA microspheres is the superficial subdermal layer, although the sub-SMAS and near-periosteal layer can be used for specific indications.

While post-treatment massage is not necessary, it is recommended to massage for the first 3-5 days after treatment to ensure even product distribution, thereby reducing the risk of nodule formation and optimizing results.

Precautions

Pain medication is usually not required, but may be recommended as needed. Maintenance treatment is typically required every 24 months, but patients with accelerated collagen degradation or severe facial aging may require a supplement every 12 months. While some effects may be noticeable in the first few weeks, treatment results should be evaluated 2 to 6 months after the last treatment.

Indications and Recommended Dosage

L-Lactic Acid Polyethylene Glycol is suitable for treating aging and sagging skin, subcutaneous tissue reconstruction, and atrophic scars.

It is also used to prevent sagging and aging, and to replace subcutaneous or deep facial wrinkles. Non-facial indications extend to areas such as the neck, collarbone, arms, thighs, abdomen, buttocks, hands, knees, and genital area.

Clinical expert members have provided detailed recommendations on the number of vials required per treatment session for different treatment areas. Generally, for facial care, 1 to 2 vials per treatment session is sufficient (Figure 1). When using two vials, the treatment area can be extended to the neck area. Figure 2 illustrates the results observed during a 4-month follow-up of a patient who received 2 vials of L-Lactic Acid Polyethylene Glycol (one vial per half of the face). Each vial was reconstituted with 7 mL of sterile water for injection and 1 mL of 2% lidocaine solution. Figure 3 depicts the volume injected into each site area.

The proposal also recommends using 1 to 2 mL of 2% lidocaine to minimize discomfort. Injection techniques include linear retrograde and fan-shaped patterns, preferably using Luer lock syringes and 25-30 gauge cannulas and needles. The solution volume per vial varies depending on the treatment area to promote uniform distribution. Carefully select appropriately sized syringes to prevent sedimentation, taking into account the expected time required to inject the contents of each syringe. Treatment indications include the treatment of aging skin and subcutaneous reconstruction; the frequency of treatment and maintenance regimen depend on the severity of the patient's condition. Special consideration must be given to bolus injections and surgeries involving the periorbital and perilipal areas; caution is strongly advised. Maintenance therapy is recommended every 24 months, with annual maintenance for more severe cases.

Treatment Frequency 

The treatment frequency is selected based on the severity of the client's condition. For mild and preventative cases, one treatment is sufficient; moderate cases may require 2 to 3 treatments; severe cases may require three or more treatments and may be combined with other treatments. The minimum interval between treatment courses should be 30 days, with the last injection scheduled at least 90 days after the penultimate injection to ensure that the final results of the previous injection can be evaluated to plan the next intervention.
